Wentworth Health Partners is a Joint Commission Accredited Organization with Primary Care Medical Home Certification. A primary care medical home isn’t a place – it is a collaborative partnership with your primary care provider. At Wentworth Health Partners, you are at the center of your care, working with a team of clinicians and support staff to create a personalized health plan to meet your goals. Everyone’s health care journey is different. As a primary care medical home, our primary care practices guide your journey and coordinate your care.    

What Does a Patient-Centered Medical Home Mean for You?

You have a highly skilled team ready to care for you. Your care team is responsible for providing comprehensive primary care and coordinating all of your health care needs. You select your primary care provider, and you may change providers at any time. You may seek a second opinion and/or specialty care at any time should you wish.

  • Pharmacists
  • Certified Medical Assistants
  • Behavioral Health Specialists

We treat you as a whole person, not just as one illness. We are concerned about all of your health needs and are responsible for coordinating your care. We will work with you and your family on:

  • A plan of care for any major health problems.

It means access 24/7.

We can provide advice quickly – be it during a visit or anytime outside of your visit. Also, you can often get appointments the same day. Call your office and you will be connected to your care team or to one of our excellent providers on-call if after hours.

It means coordination.

We organize:

  • Visits with specialists, hospital care and results from labs, imaging tests, etc.
  • The medications you take – both over the counter and prescriptions from any doctors.
  • Services you receive, such as programs to stop smoking or lose weight.
  • Your information -- A patient plan will be given to you at the end of every visit detailing the reason for your visit, medications prescribed, instructions on recommended treatment and any follow-up needs.

Our secure, web-based Patient Portal helps:

  • Keep you up to date; remind you when you need care, testing, or new information. 
  • Send prescriptions right to the pharmacy.
  • Connect you with your care team for non-emergent needs, request appointments and prescription refills; see lab results, your personal health record and track your health information. Please allow 24 hours for portal responses.
