Primary Care Medical Home

Wentworth Health Partners is a Joint Commission Accredited Organization with Primary Care Medical Home Certification. A primary care medical home isn’t just a place – it is a collaborative partnership with your primary care provider.

What is a Medical Home?

A Medical Home is an approach in which you have a personal relationship with your care team. Your treatment is coordinated through this team to ensure you receive the necessary care when and where you need it, in a manner you can understand.  

Your highly skilled team is ready to care for you and is committed to providing you with continuous, comprehensive and coordinated care demonstrated by evidence-based practices.

IN YOUR MEDICAL HOME, YOU CAN EXPECT:

Personal and Caring Team Members

We can assist you in selecting your own personal provider.

Your Care Team Includes:

  • Board Certified Physicians or Certified Nurse Practitioners/Physician Assistants
  • RN care managers
  • Certified Medical Assistants
  • Patient Service Representatives

Based on your needs, your care team may also include:

  • Pharmacists
  • Behavioral Health Specialists
  • Health Coaches
  • Social Workers and more

Access to Care – Fast

We are available when you need us 24 hours a day, 7 days a week. Call your office and you will be connected to your care team or to one of our excellent providers on-call if after hours. Our online Patient Portal is also available to request an appointment or send non-emergent messages. Please allow 24 hours for portal responses.

The Latest Technology to View Your Personal Medical Information

By using the Patient Portal, you can connect with your care team for non-emergent needs, request appointments and prescription refills, see lab results and track your health information.

Coordinated Care

Your care team is concerned about your health and is responsible for coordinating your care, such as referrals to specialists, behavioral health, hospice, home care, etc.

We organize visits with specialists, hospital care and results from labs, imaging tests, etc.

Your care team can connect you with services you want to receive, such as programs to stop smoking or lose weight.

Support to Help You Manage Your Health

We make every effort to engage you and your family in your care and health care decisions; in the development of your treatment plan; and in educating you about your health.

Understand Your Care Team

If you need the assistance of an interpreter or other assistive devices, we will arrange these services to be present during your visit.

A Summary of Your Visit – Every Time

A patient plan will be given to you at the end of every visit detailing the reason for your visit, medications prescribed, instructions on recommended treatment and any follow-up needs.

Your Role 

As our patient, we encourage you to take an active role in your care with your team.

We are here to listen and answer your questions. Please do not hesitate to ask us any questions or share any concerns, especially if it’s about safety. We want to hear what you have to say. Tell any member of our team if something doesn’t seem quite right.

Please ask us if you have questions about any medications or about the purpose of any medications or prescriptions you are given, including possible side effects.

Please be prepared and on-time for appointments. 

Carefully read forms and consents for procedures and make sure you understand them before signing. If you do not understand, ask your provider or a care team member to explain them.

Should you need to cancel your appointment, please call at least 24 hours in advance.

Ensure that your care team has your current contact information. We will need a complete and accurate personal and family history, the names of any prescribed or over-the-counter medications, the names of other health care providers you have visited, recent hospitalizations, etc.

Please bring every medication you normally take in the original containers to your appointment. This includes prescriptions, over-the-counter medications, and herbal medicines.

 You will receive a patient plan before you leave each appointment. Please review this patient plan and make sure you understand the instructions. It is also a good idea to book your follow-up appointment or know when your provider expects to see you next.
