Medical Records

Wentworth-Douglass maintains detailed, private records about your healthcare. Patients or their personal representative may review or obtain copies of their Wentworth-Douglass Hospital medical records or request copies to be sent to another facility.

Release of Information Request

You may request copies of your complete personal health records by calling our Medical Records office at (603) 740-2591, Monday through Friday 8:00 a.m. – 4:00 p.m.

A Protected Health Information Release Form (PDF) must be filled out by everyone requesting medical information. The release form must be filled out completely and signed in order to be processed. 

To return the completed form, you may either: 

  • Bring it to the Medical Information Department, located at 121 Broadway, Dover, NH. Our office is located on the 2nd floor. 
  • Or, fax it to (603) 740-2290. 

Requesting Images on CD

To obtain a copy of your Imaging study, please contact the Imaging Services File Room at (603) 740-2588. Please allow 48 hours to fulfill the request. All Imaging CD requests will be available for pick up at the Medical Information Department located at 121 Broadway, Dover, NH. 

You may also download the CD Request form and fax the request to us at (603) 740−2650. 

When requesting Medical Records, please consider these guidelines:

  • When possible, please give our Medical Records Office a 48-hour notice prior to picking up information.
  • Please indicate if there is specific information you need or a date of service (ex: a Nuclear Medicine study done on 1/25/2015).
  • You must have two forms of identification before leaving with any medical information. This can include a driver’s license, picture ID and another ID with a signature, such as a credit card, library card or Social Security card.
  • If you are picking up information for another person, you must also have two forms of identification and a written note from the patient requesting information with a written indication of who will be picking up the information. The patient requesting the information must sign the note.
  • Only Legal Handwritten Signatures are accepted.
