At Wentworth-Douglass Hospital, our Case Management team will work closely with you and your insurance company to ensure that your proper discharge planning is in place. Patients may either return home with VNA services, or attend a Rehab facility according to insurance approval and safety guidelines. These options will be discussed with you on an individual basis and according to your needs before discharge. At Wentworth-Douglass Hospital our goal is to assist you through this journey of wellness and return you to optimal health.
